Nothing is missing, paste this code into your html file inside <body> element and include bootstrap.css you downloaded. Assumption is that you downloaded Bootstrap :)

Maybe its because im using a custom bootstrap?

This HTML code is written for Bootstrap 2. If you use Bootstrap 3.0, you'll have to use "col-md-3" instead of "span3" class. That is only difference. Classes "container", "row" and "span" are generic, I don't think you don't have them, even if you customized Bootstrap. Make sure you have included the path to your bootstrap.css (or bootstrap.min.css or bootstrap-responsive.css) before the </head> tag in the HTML file.
